Overview Fairview Health Services is looking for a Visti Facilitator to add to our team. The Visit Facilitator will support the Midway Clinic in St. Paul. clinic providing superior service with their interactions, welcoming patients, ensuring high quality care with every patient. This position is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patients medical record.
